A tracheostomy is a surgical procedure in which an opening is created in the trachea (windpipe) to allow air to enter the lungs. It is typically performed when there is an obstruction in the airway or when long-term mechanical ventilation is required. In children, tracheostomy may be needed for various medical reasons and is often performed in emergency situations or when prolonged respiratory support is necessary.

Tracheostomy is less common in children than in adults, but certain medical conditions and emergencies may still necessitate the procedure.

Depending on the underlying cause, the tracheostomy may be performed electively or as an emergency. In experienced centers, it can be performed percutaneously—using a needle and camera guidance—rather than via an open surgical approach. This minimally invasive method reduces the risk of life-threatening complications associated with open surgery.

Although rare, complications such as bleeding, infection, or perforation of the trachea or esophagus may occur during or after the procedure. One of the most concerning issues for families is displacement of the tracheostomy tube, especially in the early postoperative period. This is more common after open procedures and less frequent following percutaneous techniques.

Families must be thoroughly educated on tracheostomy care, as they are often responsible for home-based care using home ventilator support after the child is discharged. Therefore, it is essential that the procedure be performed in a medical center with a multidisciplinary team, including pediatric pulmonology, intensive care, and pediatric surgery specialists, and that families are adequately informed about the postoperative care process.

Tracheostomy can be a life-saving and effective intervention for managing airway problems, but meticulous surgical technique and postoperative care are crucial. Early diagnosis and appropriate management help minimize complications and ensure the overall well-being of the child.
